Preheat your oven to 375°F (190°C).
In a large bowl, combine ground chicken, breadcrumbs, milk, egg, salt, black pepper, garlic powder, onion powder, and Dijon mustard. Mix until well combined.
On a piece of parchment paper or plastic wrap, flatten the chicken mixture into a large rectangle.
Place the slices of Swiss cheese evenly over the flattened chicken mixture, followed by the ham slices.
Carefully roll the meatloaf from one end to the other, using the parchment paper or plastic wrap to help guide it. Transfer the rolled meatloaf to a greased baking dish.
Sprinkle the grated Parmesan cheese evenly over the top of the meatloaf.
Bake in the preheated oven for 45-50 minutes, or until the meatloaf is cooked through and the top is golden brown.
Let the meatloaf rest for a few minutes before slicing. Serve hot.
