In a medium bowl, add the peanut butter, soy sauce, rice vinegar or lime juice, honey or brown sugar, sesame oil, grated ginger, and minced garlic.
Stir together using a whisk or fork until the mixture is smooth and creamy.
Slowly stir in the coconut milk or water until you reach your desired consistency.
Taste and adjust. Add lime for more tang, honey for sweetness, or Sriracha/red pepper flakes for spice.
Transfer to a serving bowl and top with chopped peanuts and cilantro if using. Serve immediately or store for later.
