Add all of the ingredients to your slow cooker.
Stir very well to combine.
Cook on low for approximately 6 hours, or until thick (this time may vary based on your slow cooker).
Stir several times throughout the cooking process to prevent sticking.
If it hasn't thickened enough, remove the lid and continue cooking until desired thickness is reached.
Allow to cool and store in the refrigerator for 2 to 3 weeks, or freeze for up to a year.
