In a medium saucepan, combine the heavy cream and sugar. Heat over medium heat, stirring until the sugar is fully dissolved. Bring to a gentle simmer, but do not let it boil.
Remove from heat and stir in the lemon zest and lemon juice. Mix thoroughly to infuse the flavor.
Let the mixture cool for about 10 minutes, then pour into ramekins or dessert glasses.
Cover with plastic wrap and refrigerate for at least 4 hours or until fully set.
Garnish with fresh berries and mint leaves if desired. Serve chilled.
