Preheat the oven to 350°F. Line a baking sheet with parchment paper.
In a large bowl, combine all loaf ingredients. Stir thoroughly with a rubber spatula or clean hands until fully mixed.
Transfer the meat mixture to the prepared pan and shape into a loaf about 12 inches long and 4 inches wide.
Bake uncovered for 40 minutes.
While the loaf bakes, mix all glaze ingredients in a small bowl.
Remove the meatloaf from the oven, increase temperature to 375°F, and spread half of the glaze over the top.
Return to the oven and bake for 20 more minutes, until caramelized and the internal temperature reaches 160°F.
Remove from oven, rest briefly, slice, and serve.
